Common MDAC mistakes to watch for

Small errors cause delays at check-in or immigration. Avoid these.

Entering your patronymic (-ovich/-ovna) in a name field. The MDAC form has no middle name field. Enter given name and family name only.
Copying the passport order into MDAC. Uzbek passports list surname first in caps, but MDAC expects given name first. Check which field is which.
Entering the wrong departure city. If you transit through Istanbul, your last port before Malaysia is IST, not TAS. Many Uzbek travelers list Tashkent instead.
Using the wrong date format. Uzbekistan uses DD.MM.YYYY. The MDAC form expects DD/MM/YYYY or YYYY-MM-DD. A dot instead of a slash can cause problems.
