Your hotel name and address for your first night. Malaysian addresses use street name, city, postcode, state format. If you are visiting for medical tourism, use the hospital or clinic address you are registered with.
Common MDAC mistakes travelers from Nepal should avoid

- Omitting your middle name on the MDAC form when it IS on your Nepali passport. If your passport says Ram Bahadur Thapa, enter Ram Bahadur as your given name. Leaving out Bahadur creates a name mismatch that immigration will flag.
- Confusing given name and surname. Many Nepali names have compound given names. If your name is Suman Kumar Shrestha, Suman Kumar is the given name and Shrestha is the surname. Put them in the correct MDAC fields.
- Believing the eVisa makes the MDAC unnecessary. The eVisa grants permission to enter Malaysia. The MDAC registers your arrival electronically. Immigration checks both documents before allowing entry. Having one without the other means delays at the counter.
- Submitting the MDAC too early. The 72-hour window is strict. If you submit a week before your flight, the system may reject it and you will have to redo it closer to your departure date.
