Some Mozambicans also use Bantu names, which may include a clan name or a single name with no family name.

The MDAC form has one surname field and one given name field. There is no space for a middle name or a second surname. If your name is Carlos Alberto Mendes, put 'Carlos' in the given name field and 'Mendes' (your father's surname) in the surname field. Leave out your mother's surname. This trips up a lot of Mozambican travelers.

If you have a Bantu name with multiple parts, enter the main name you use in the given name field and the clan or family name in the surname field. The key rule: look at your passport and copy exactly what is written there. Your MDAC must match your passport.

Portuguese name structure

Given name + mother's surname + father's surname. MDAC has one surname slot. Enter your father's surname only. Example: For 'Joao Alberto da Silva', surname is 'Silva'. Do not put both surnames.

Bantu names and dates

Some Mozambicans use Bantu naming traditions with a single name plus a clan name. Enter the clan name as your surname. For dates, Mozambique uses DD/MM/YYYY in Portuguese format. That matches MDAC's format perfectly. No conversion needed.

Common MDAC mistakes Mozambican travelers should avoid

These are the problems we see most often from travelers coming from Maputo.

  • Thinking the eVisa is enough. The eVisa and the MDAC are two different things. You need both. Some Mozambicans finish the eVisa and stop there. The MDAC is a separate step you must also complete before arrival.
  • Putting both surnames in the MDAC surname field. The MDAC has one surname field. If your name is Ana Cristina Fernando, put 'Fernando' only. Do not put both 'Cristina' and 'Fernando' together. The system reads them as one long last name.
  • Forgetting the 6-month passport rule. Your Mozambican passport must be valid for at least 6 months from the date you enter Malaysia. Some travelers check this too late and have to cancel their trip. Check your passport before you book anything.
  • Bantu names with multiple parts causing confusion. If you come from a community that uses a first name plus a clan name, do not try to split the clan name into middle and last name. Enter the clan name as your surname. Keep it simple.
