The MDAC form has two name fields: given name and surname. If your surname is Grimaldi-Lambert, enter the complete double-barrelled surname with the hyphen in the surname field. Do not split it into two names or drop the second part. If your surname includes de (like de Polignac), include it in the surname field as shown on your passport.
