Lithuanian names use special diacritical marks: ą, č, ę, ė, į, š, ų, ū, ž. The MDAC form may not support these characters. If your name on your Lithuanian passport uses them, enter the plain Latin version instead: a, c, e, e, i, s, u, u, z. Some Lithuanian passports already show names without diacritics, so check yours first.
Lithuanian surnames change depending on gender and marital status. A man named Paulauskas may have a wife named Paulauskienė and a daughter named Paulauskaitė. These are not different family names. They reflect the same root with a different suffix that shows if the person is male, married female, or unmarried female. This is important on the MDAC form. Enter your surname exactly as it appears on your passport, not how your spouse or child spells theirs.
There are no patronymics in Lithuanian naming. But the suffix tells you something about the person: -as, -is, -us are typically male, -a and -ė are typically female, -ienė means married woman, and -aitė or -utė means unmarried woman or daughter.
