The MDAC form has one surname field and one given name field. Libyan passports show your name in both Arabic and Latin script. Use the Latin transliteration exactly as it appears on your passport. If your name is written as Ali Al-Mahdi, enter Ali as your given name and Al-Mahdi as your surname. Do not change the spelling, remove hyphens, or add spaces.
Many Libyan names include Al- or Bin prefixes. These are part of your name and belong in the surname field. For example, if your passport says Omar Bin Khalid, Bin Khalid is your surname.
