Address in Malaysia

The name and address of your first hotel. Lao addresses use Ban (village), district, province format. Malaysian addresses use street, city, postcode, state. Write your hotel address in the local style.

Common MDAC mistakes travelers from Laos should avoid

Catching these early saves you time at immigration.

  • Mixing up your surname with your given name. Many Lao family names are short, like Souk, Keo, or Boun. If your Lao passport lists Boun Somsy, then Boun is the surname and Somsy is the given name. Do not reverse them on the form.
  • Typing your address in Lao script. The MDAC form works with Latin letters only. Convert your home address from Lao to English before you start typing.
  • Skipping MDAC because you are visa-free. Visa-free entry and MDAC are two different rules. Immigration checks both. No MDAC means no entry even with a valid Lao passport.
  • Entering an old passport number by accident. If your Lao passport was recently renewed, use the current travel document. The system compares against the number on your new passport.
