Copying the passport name order into MDAC. Kazakh passports list surname first in capitals, but MDAC expects given name first. Check the fields carefully.
Entering Cyrillic characters into the MDAC form. The system only accepts Latin letters. Use the Latin transcription from the machine-readable zone of your passport.
Spelling mismatch between airline ticket and passport. Kazakh names can have multiple valid Latin spellings due to Cyrillic transliteration (Q vs K, AE vs A). The name on your ticket must match your passport.
Using the wrong date format. Kazakhstan uses DD.MM.YYYY. The MDAC form expects DD/MM/YYYY or YYYY-MM-DD. A dot instead of a slash can cause problems.
