- Mixing up Nom (surname) and Prénoms (given names) on the MDAC. French-format Congolese passports show Nom first, then Prénoms. On the MDAC form, you enter Given Name first and Family Name second. - Assuming MDAC replaces your visa. It does not. Congolese passport holders need a Malaysian visa. The MDAC is an extra requirement, not a substitute. Apply for your visa weeks ahead, then submit MDAC in the 72-hour window before you fly.
- Entering a Congolese home address instead of Malaysian accommodation. The MDAC form asks for where you stay in Malaysia, not your address in Kinshasa, Lubumbashi, or Goma. Provide your hotel, guesthouse, or business address in Malaysia.
